Requirements: RN Heals 4 Region 4a

Letter of Intent should be addressed to the Region 4a Director,

 

 

EDGARDO GONZAGA, MD, MSc, CESO III
Director IV-A

  1. Resume
  2. TOR
  3. Diploma
  4. License
  5. Board passing and rating
  6. other certificate

Take note: You have to secure 3 copies.

    Taga Cainta po ako, kasama ba yun sa Region 4-A? Oct 29 po ako mag aapply. Pwede kaya yun even without the application form. Thanks!

      hey cainta, rizal is aso a part of Region 4a- calabaRzon. the application form is already available at http://www.doh.gov.ph u can download it now. 🙂

    pwede pa po ba magpasa? hindi ko pa po kasi masyadong alam tong RN Heals, ang tanong ko pa po is kung saan po ba nagpasa ng requirements eh dun ka na rin maaassign? taga cavite po kasi ako. Thanks nurse germz!

    Good day..may i ask if wen the RN Heals Batch 4 in Quezon will start? How about, the requirements of PRC License…I am one of the new passer of June-July 2012? 4 to 6 months before my Prc license will release..If I have my claim slip of registration to Prc will be valid? Thanks for the possible response..

      mejo strict ata when it comes to PRC ID kasi kailangan ipapakita yan. but if pagpipilian mas valid po ung certificate kaysa sa claim slip po, andon na kasi ang prc number. hope for the best nlng na tatanggapin nila
